But for Evangelista, it led to a rare and devastating side effect called paradoxical adipose hyperplasia (PAH). Instead of shrinking fat cells, PAH causes them to grow and harden, creating large, painful masses beneath the skin.
In her candid interview with Harper’s Bazaar, Evangelista revealed the extent of the damage: she underwent multiple liposuction surgeries to remove the hardened fat, suffered from collapsed lungs, and was left with significant scarring. She even needed corrective surgeries under her chin alone four different times. What was intended to be a minor tweak ended up drastically altering both her body and her life.
Evangelista later sued Zeltiq Aesthetics Inc., the parent company of CoolSculpting, for $50 million; the case was ultimately settled out of court in July 2022.
The physical injuries were just part of the trauma. Evangelista described feeling “brutally disfigured” and slipping into a deep depression. She became so isolated and ashamed that she largely withdrew from public life for years. According to Fox News, the experience led her to therapy to address the emotional scars CoolSculpting had left behind.
